When you need it
After the preliminary review, the consulate tells you which country and how many past years of travel history to provide. Fill it in as stated in the notice, listing each trip with country, dates, length of stay and purpose.
Please read before you start
- Formatting only. Your answers are printed onto the official form exactly as typed; we do not verify or guarantee their accuracy.
- The signature fields are blank. Print the form and sign by hand (a guardian signs for applicants under 16 as the form explains). Electronic signatures are not accepted.
- Acceptance is up to the consulate. We cannot guarantee the form will be accepted; always follow the consulate's latest notice.
- Please be truthful. False or incomplete information may lead to visa refusal or denial of entry; the applicant bears the consequences.
- Used only to generate, then deleted. What you enter is used only at the moment the PDF is generated and is deleted from server memory immediately afterwards — never stored in a database, never logged, no copy kept.
